Mr. Phillip Collins, Jr. passed away unexpectedly on Wednesday, August 29th, 2018, at Holy Cross Hospital, Chicago, Illinois.
Public Viewing & Visitation will be held on Friday, September 7th, 2018.
Visitation: 2:00 p.m. - 8:00 p.m.
Family Hour: 6:00 p.m. - 8:00 p.m.
A celebration of Life followed by Funeral Service will be held on Saturday, September 8th, 2018.
Celebration of Life: 10:00 a.m.
Funeral Service: 11:00 a.m.
All services will be held at:

Interment immediately following all services at Cedar Park Cemetery, Calumet Park, Illinois.
