Phyllis Jean Collins-Jones, age 49, passed away Saturday, March 9, 2013 at Seasons Hospice at Weiss Hospital, Chicago, Ill, with her family at her side.
Viewing and Visitation will be held at Golden Gate Funeral Hone, 2036 W. 79th Street, Chicago, Ill, on Friday, March 15, 2013, 4:00 p.m. - 8:00 p.m.,; Family Hour will be held from 6:00 p.m.-8:00 p.m.
Funeral Services will be held on Saturday , March 16, 2013, at 11:00 a.m. at Holiness Is The Way COGIC, 8210 South Racine Avenue, Chicago, Ill.
Interment Will Be Private.
